Ergometry, also known as an exercise test or stress test, is a fundamental non-invasive cardiological diagnostic method. The primary goal of this test is to identify abnormalities that are not visible on a resting electrocardiogram (ECG).
Although the heart at rest may have normal blood flow even when there are significant narrowings in the blood vessels, physical exertion causes the heart to pump faster and more forcefully, thereby increasing the demand for oxygen. If a condition such as coronary artery disease is present, the heart muscle will not receive adequate blood supply, which becomes evident through specific ECG changes or the onset of symptoms.
Doctors opt for ergometry when a patient reports symptoms such as chest pain, shortness of breath, unexplained dizziness, or palpitations, especially if they occur during exertion. This test is also used to evaluate the effectiveness of therapy in patients with existing heart conditions, as well as to assess cardiac status before certain surgical procedures.
Proper preparation for an ergometric test is crucial for obtaining accurate and reliable results, as well as for patient safety. For several hours before the test, typically two to three hours, you should not eat, drink, or smoke. Consuming a large meal immediately before the test can place additional strain on the body and affect the results, and it may also make exercising uncomfortable.
It is especially important to avoid all products containing caffeine for at least 24 hours before the test. This includes coffee, various teas, energy drinks, and chocolate. Caffeine, as a stimulant, can accelerate the heart rate and raise blood pressure, which directly interferes with the interpretation of the results.
Equally important is consulting your doctor about taking your regular medications. Certain drugs, particularly beta-blockers that slow the heart rate, may need to be temporarily discontinued. Patients should never make such decisions on their own but only follow the cardiologist's instructions.
On the day of the test, it is essential to wear comfortable, light, and layered clothing that allows freedom of movement, such as sweatpants or athletic pants. The test is stopped when the target heart rate is reached, if significant ECG changes occur, if the patient experiences symptoms such as severe chest pain, pronounced shortness of breath, or dizziness, or simply if they can no longer continue due to fatigue.
The exertion itself typically lasts between eight and fifteen minutes, and the entire procedure, including preparation and recovery, takes about an hour.
After the exertion phase, a recovery phase follows, lasting several minutes. The patient is asked to rest while heart rate and blood pressure return to near baseline, and the ECG continues to be monitored. The test result can be negative, meaning that during the given exertion, no changes were observed that would indicate reduced blood flow to the coronary arteries.
Conversely, a positive result suggests the possibility of coronary artery disease and typically requires further testing, such as coronary angiography, to confirm the diagnosis and determine the degree of arterial narrowing. Ergometry is considered a very safe test in most cases. Complications are extremely rare and may include irregular heart rhythms or a drop in blood pressure, while a heart attack is exceedingly rare. The entire process is conducted under the careful supervision of experienced medical staff who are prepared to intervene immediately in case of any difficulties, thereby minimizing risk to the lowest possible level.
